Troop Four Permission Slip
For the Troop Four camping trip to Mount Monadnock State Park in Jaffrey NH on April 9th – 10th, 2011

Permission Slip is due at no later than the start of the March 31st troop meeting. Call Phil Bedard before March 31st at 508-561-7058 or pbedard1@gmail.com if your scout would like to attend the day hike trip, but can’t turn in this form at the start of the March 31st troop meeting.

There is a nominal $5 fee per person to offset the cost of the state park campground fee.  Please make check payable to Troop 4 and attach the check to the permission slip.  If cash, please be sure it is in an envelope with your scout’s name on it.
